What to Expect at Your Annual Check-Up

At your annual check-up, our providers will take a detailed health history and family health history, including any recent surgeries or medication changes that have been made. We urge our patients to take this time to discuss any concerns you may have about your health with us. We will also review your lifestyle habits such as smoking, alcohol consumption, diet and exercise. At Total Point Prescott Medical, we offer weight management counseling and health & wellness counseling to help you if you would like to modify your lifestyle habits. In addition to annual checkups with your primary care provider, your life may require specialized physicals to ensure your wellness for various activities. Total Point Prescott Medical offers specialized physical exams for all of your needs.

What We Check

No matter the type of physical you or your child may need, there are a few specific things our providers will always check. These things include:
Sports Physicals

School sports and recreational leagues generally require an up-to-date physical to participate in a sport. The physical will also provide medical instructions for children with special health conditions, such as asthma or a life-threatening food allergy. A sports physical will help determine if your child meets the health requirements to participate in a sport. As a patient of Total Point Prescott Medical, we will have the necessary records to help complete your child’s sports physical. During the physical assessment, we will collect the following health information:

  • Previous Fractures, Sprains or Muscle Strains
  • Heat-related Illness Events
  • Asthma History and Exercise-induced Asthma Flares
  • Fainting History
  • Concussion History
  • Medication including Over the Counter Medicines

These examinations can help determine if your child has any underlying conditions that may affect their health as they participate in a sports activity. If your child sustains an injury at a sporting event, a completed sports physical will help a medical professional treating your child to make a more accurate diagnosis.

Work Physicals

Total Point Prescott Medical is pleased to offer work physicals and screenings for patients needing to comply with company procedures and regulatory requirements. If you are a patient, we will have your up-to-date health records to help complete the process. Total Point Prescott Medical provides general work physicals, as well as return to work and pre-employment physicals.

If your company requires specialized screenings for employment, we can also provide:

Total Point Prescott Medical will assess your overall health and look for any symptoms of illness or chronic disease that may affect how you perform the duties of your job. We will record your medical health history, current list of medications and perform a physical exam.

It will be very helpful for you to bring any paperwork provided by your employer so that we can fill out the information at the time of your work physical. School Physicals

A school physical will help determine if your child meets the health requirements to start school. As a patient of Total Point Prescott Medical, we will have the necessary records to help complete your child’s school physical. During the physical assessment, we will collect the following health information:

  • Asthma History
  • Fainting History
  • Concussion History
  • Medication including Over the Counter Medicines

These examinations help determine if your child has any underlying conditions that may affect their health and that they meet all requirements of the school.

Camp Physicals

Summer camps generally require an up-to-date physical to attend. The camp physical includes current immunization records as well as any medications that your child may require, and a synopsis of their preventative care. The physical will also provide medical instructions for children with special health conditions, such as asthma or a life-threatening food allergy. Our pediatric specialists at Total Point Prescott Medical offer camp physicals for your children.

 

During the physical assessment, we will collect the following health information:
