How Can We Help You?

Yes. While we can’t accommodate or give out every kind of medical care, you do need to make an appointment to come in for all of the conditions that we treat.

You can make an appointment anytime during our normal business hours of operation.

Upon receipt of a signed release form, your former practice will forward your records to our office.  It is best to contact your former office for their form, since their release policies and prices will be disclosed to you.  We usually do not need all of your records, generally the last couple of years will suffice, unless you have a complicated medical history.

We will provide the medical record release form for which to give to your current provider.

We accept many insurance providers. For a current list of carriers, please view our Billing & Insurance page.

For your visit with us, be sure to bring the following:

  • Driver’s License
  • Insurance cards (including any drug coverage cards)
  • All medications
  • Any records from former doctors that you may possess

Yes! We see patients on a self-pay basis.

We ask that you call the office and let us know what time you will arrive. We will check with the providers to see if you can be seen at that time or if it would be better to reschedule.

We expect you to call us to cancel your scheduled appointment at least 24 hours in advance so that we have a chance to offer that time slot to another patient.

Please call us to make an appointment so we can discuss your results in person.

Total Point Prescott Medical offers some weekend appointments, but if the patient is having a life threatening medical emergency, then they should call 911 or proceed to the closest Emergency Room or Urgent care facility.

No need to prep, just come in for your appointment.
